Mental Status Exam Builder and the Risk Baseline

A mental status exam is not a form, it is a place to put what you actually saw. File each observed finding into its domain and a real note assembles itself on the right — including the two findings that should stop the interview and change the differential. Then do the harder half: separate this person’s chronic baseline from acute-on-chronic escalation, and build a safety plan rather than asking for a promise.

File the findings

Ms. R, 27, is on the behavioural unit with borderline personality disorder and a co-occurring depression. Everything below is something you saw or heard in one twenty-minute interview. Put each into the domain it belongs to. Two of them are not personality findings at all, and the note will tell you which.

Baseline, or above baseline?

Documented baseline — from her chart and her own accountDaily passive thoughts of dying for four years. Two overdoses, the most recent three years ago. Superficial forearm self-injury roughly monthly. Lives with her sister. Weekly outpatient therapist. She describes this as “how it always is”.

The most informative data are not items on the form. They are the interpersonal field — how fast the person becomes close, how sharply they turn, and how you feel twenty minutes in. Wanting to rescue, wanting to be the good nurse, wanting the interview to end: that is countertransference, it is clinical information about the person’s relational world, and it is not a personal failing. Name it in supervision. Never act on it at the bedside.

Where the risk actually sits

CHRONIC BASELINE Sort the items on the left
Chronic risk is not the emergencyMany people with borderline personality disorder carry a persistent baseline of suicide risk that admission does not resolve, and admitting for chronic risk alone can worsen outcomes — it can reinforce crisis as the route to care, interrupt the therapy that works, and expose the person to iatrogenic harm. What warrants acute action is escalation above that person’s own baseline. Sort the items and the count appears here.

Build the safety plan

Select every element that belongs in her written safety plan. Four of these are not safety planning, however familiar they look. Choose, then check.
